Description

Background: Neural power spectra contain an aperiodic $\frac{1}{f^\alpha}$ background and superimposed oscillatory peaks, but conventional band-power metrics do not separate these contributions. The FOOOF (Fitting Oscillations & One-Over-F) algorithm \cite{1} provides a parametric decomposition of both components, enabling analysis of spectral features associated with distinct physiological processes. Here, we apply this framework to electroencephalographic (EEG) recordings during emergence from general anesthesia.

Methods: We retrospectively analyzed frontal EEG recordings from 306 patients previously classified into six emergence trajectories based on alpha and delta power \cite{2}. Absolute and relative alpha and delta power were calculated and the FOOOF algorithm was applied to extract the spectral exponent and alpha peak height. All spectral metrics were time-normalized to allow group-level comparisons.

Results: Mapping the emergence trajectories in the alpha-delta plane revealed trajectory-specific patterns, however, they lacked convergence at emergence endpoints. In contrast, FOOOF-derived parameters demonstrated a progressive flattening of the spectral exponent and a decrease in alpha peak height across all trajectories.

Conclusion: While alpha and delta power distinguish emergence trajectories, the spectral exponent consistently tracks neurophysiological transitions during anesthesia emergence, suggesting a trajectory-independent marker of awakening dynamics.
